Qualifications to enroll with Cornerstone House Calls

To enroll in Cornerstone House Calls, you must be UNABLE TO TRANSPORT YOURSELF to a primary care provider.

Counties Serviced:

Collin, Dallas, Denton, Ellis, Grayson, Hunt, Kaufman, Rockwall, Tarrant

Cornerstone House Calls accepts referrals from the following:

•  Self-Referral
•  Home Health
•  Nursing Home/Senior Center/Other Care Facility
•  Primary Care Physician

Upon enrollment, you will be assigned a Nurse Practitioner in your area. Our Nurse Practitioners make every effort to see new patients within 7 days of your enrollment. If you are referred by a hospital or other care facility, visits will be conducted within 72 hours of enrollment.

Once enrolled, you will receive a visit from your assigned Nurse Practitioner every 30-90 days in an effort to manage your health care needs.

Making an Appointment

Cornerstone House Calls hours of operation are from 9:00 a.m. to 5:00 p.m.
