What exactly is house calls?

A house call is a visit to the home of a patient or client by a clinician. If it is difficult for you to visit a traditional doctor’s office, Cornerstone house calls may be right for you.

What area do you service?

We serve the DFW metroplex and surrounding areas including Collin, Denton, Ellis, Grayson, Hunt, Kaufman, Rockwall and Tarrant.

Do I qualify to receive at home visits?

If you have difficulty getting to a doctor’s office, then in home healthcare visits are likely covered by your insurance. If you are home bound, use a wheelchair, walker, have physical or cognitive challenges when leaving home, or need the assistance of another person to leave home, you will often qualify as homebound. Contact your health plan to confirm your eligibility.

Do I need a referral for a house call?

Not necessarily, talk to your health plan to determine if you need a referral for our services. Physicians, nurses, care givers, and facility managers can also call and refer their patients. You can also complete the New Patient Registration Form on the Patient Resources page to make registration faster.

Does Cornerstone House Calls provide emergency medical care?

No, we will see you as quickly as we can, but this is not the same as emergency care. If you are experiencing a medical emergency, please dial 911 or go to the hospital. Although, feel free to contact Cornerstone House Calls at any time and we will schedule a visit with you as soon as possible or provide a phone consultation.

How often will I be seen?

You and your clinician will determine the appropriate frequency of visits that are medically necessary to manage chronic illnesses, but you can usually expect visits every 30-90 days. If you need to be seen prior to your next scheduled visit, call us at 972-798-8001 to schedule an appointment.

Is Cornerstone House Calls accepting new patients?

Yes, we are currently accepting new patients. Please call us at 972-798-8001 to schedule an appointment or fill out our online registration form.

What are your hours of operations?

Appointments are available Monday through Friday between 9:00 AM -5:00 pm CST. Call us if you need a visit outside of these hours and we may be able to accommodate you depending on schedules. We are closed on weekends and holidays.

Will a house call physician replace my primary care physician?

We can treat you for a single episode, on transitional basis, or assume the role of your primary care provider. It is your choice. Usually our patients enjoy the convenience and quality of Cornerstone House Call visits and ask us to become their Primary Care Provider.

Will I be treated by the same person?

We make every effort to send the same clinician to visit you for each house call. On rare occasions, you may be seen by another clinician, but this is very infrequent.

What mobile services are available?

Our services include the same health care services you would receive in a traditional doctor’s office, including physical examinations, laboratory services, diagnostic testing, prescriptions and mobile x-rays. In addition, we coordinate care among other providers, including home health agencies, medical equipment and social service providers.
